Do changefreq and priority matter?

Google says it ignores both; lastmod is the field it uses, and only when it’s kept accurate. Other engines still read them, so the tool includes them.

Where do I reference the sitemap?

Add Sitemap: https://yoursite.com/sitemap.xml to robots.txt and submit the URL in Google Search Console and Bing Webmaster Tools.
